Psychoneuroimmunologie bipolarer Erkrankungen:
Neue Erkenntnisse und mögliche Therapieansätze
Abstract
Abstract
Das Spektrum bipolarer Erkrankungen zeigt deutliche Zusammenhänge mit systemisch inflammatorischen
Veränderungen des Immunsystems. Neben einer genetischen Prädisposition scheinen auch Umwelteinflüsse wie Infektionen,
Autoimmunerkrankungen und chronisch entzündliche Prozesse eine pathophysiologische Relevanz zu besitzen.
Sehr wahrscheinlich stellen vor allem frühe Phasen der Gehirnentwicklung während der Schwangerschaft und in der
frühen Kindheit eine für längerfristig wirksame Veränderungen besonders vulnerable Zeit dar. Im Laufe der Erkrankung
können entzündliche Prozesse auch zur kontinuierlichen Schädigung des ZNS beitragen. Eine besondere Rolle kommt dabei
dem Tryptophan-Metabolismus zu. Tryptophan ist eine essenzielle Aminosäure und Vorstufe von Serotonin und Melatonin.
Proinflammatorische Zytokine führen jedoch zu einem Abbau von Tryptophan zu Kataboliten, die eine Vielfalt schädlicher
Wirkungen entfalten können. Neben einer Störung der Balance im gesamten Monoaminsystem kann es durch diese auch zu
einer Beeinträchtigung der mitochondrialen Funktion und des Glucosehaushalts kommen. Glutamaterge Kataboliten können
zu Exzitotoxizität und Apoptose führen, durch Wirkung an nikotinergen Rezeptoren können kognitive Störungen entstehen.
Auch eine Veränderung des zirkadianen Rhythmus kann resultieren. Theoretisch stellt daher der Tryptophanmetabolismus
einen möglichen Angriffspunkt für zukünftige therapeutische Interventionen dar. Bereits jetzt wurden erfolgreich
antiinflammatorische und immunmodulatorische Substanzen eingesetzt.

Publishing in high impact journals is critical to build one's career: recruitment, promotion and
funding are dependent to a large degree on an outstanding bibliography. How does one approach and achieve
publication in top journals? The cut off's for acceptance are so high that there may be very little, if any,
difference between material that is accepted and that gets rejected. How can you position your work optimally
to stand out from a large number of other submissions, so that it is sent out for peer review and eventually
accepted? The Editor of Molecular Psychiatry, Nature Publishing Group (Impact Factor of 15.470,
the top in psychiatry worldwide), will provide insights into what it takes to get outstanding research
published in top journals, drawing from his experience both as editor as well as highly-cited author in
endocrinology and psychiatry.
